We file and accept assignment of benefits for many insurance plans. These plans include but are not limited to the following:
Medicare Part B, Iowa Title XIX, HAWK-I, and Blue Cross Blue Shield. We cannot guarantee payments on your claim as your insurance is a contract between you and your insurance company.
At this time, we are not providers for the following insurance plans:

If you have membership with these plans, you may pay Vision Center P.C for your services and purchases and be reimbursed by the plan according to the out-of-network plan provisions.
